The same "Attestation du statut de résident du Québec" form also applies to a student who resided outside Canada for part of the reference period (e.g. a study, work or religious-mission stay abroad) and who must nonetheless prove the continuity of their Québec residence.
When the usual supporting documents (lease, utility bills, etc.) do not cover that period abroad, the form accepts a sworn declaration explaining the reasons for and duration of the stay outside Canada, signed before a commissioner for oaths.
The oath, in brief
This document contains a declaration under oath (or a solemn affirmation): it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, clerk, or justice of the peace.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
