Form CF-FC 2016 is used by contributors (serving or retired) to the Canadian Armed Forces pension plan who want to provide official information about their common-law union.
It is used, in particular, to allow a common-law partner to receive a survivor pension. The document is sent to the Pension Centre with other evidence of the conjugal nature and length of the relationship. The declaration must be made under oath before a person authorized by law.
The oath, in brief
Form CF-FC 2016 contains a declaration under oath or a solemn declaration: it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, or justice of the peace. Once sworn, the declaration has the same weight as a statement made before a court.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
