Form CRQ-003-1, the "Declaration to Ensure the Non-Interruption of Benefits," is sent periodically by Retraite Québec to people receiving a benefit from the Québec Pension Plan or a public-sector pension plan. It must be completed, signed, and returned within the prescribed time limit, or benefit payments may be suspended.
The declaration must be signed in the presence of a witness recognized by Retraite Québec, who completes and signs section 3 of the form themselves. Accepted witnesses include a commissioner for oaths, a notary, a lawyer, a physician or pharmacist, a police officer, a mayor, a member of the clergy, or authorized staff of a financial institution, an embassy, or Retraite Québec.
The oath, in brief
Form CRQ-003-1 contains a declaration under oath (or a solemn affirmation): it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, clerk, or justice of the peace.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
