The "Attestation du statut de résident du Québec" form, for CEGEP students, is used to establish Québec residency for tuition purposes. Among the 14 situations listed in its Part 2, one concerns a student whose resident status flows from that of their common-law partner.
In that case, the form requires, in addition to the partner's proof of residence, a sworn declaration attesting to the cohabitation (document 14 on the list of supporting documents) — to be signed before a commissioner for oaths, as set out in document 21 of that same list for sworn declarations generally.
The oath, in brief
This document contains a declaration under oath (or a solemn affirmation): it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, clerk, or justice of the peace.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
Where to get the document
This document is published in French only by Directeur des poursuites criminelles et pénales (DPCP). No official English version of this specific document is currently published — the organization's English page lists the same category of forms.

Have this document sworn online
The required sworn declaration can be signed online by videoconference: show valid photo ID, describe the cohabitation with your common-law partner before the commissioner, then receive the signed document to attach to the rest of the file sent to the CEGEP.
Before booking, confirm with the court registry or the receiving organization the deadlines that apply to your file.
