Parental authorization template
A parental authorization lets a parent or guardian authorize another person to act for their child in a specific situation: accompanying them to an activity or a medical appointment, or looking after them during an absence.
You describe the purpose of the authorization and its duration. For travel abroad, use the statement of consent for a child's travel instead.
What is this template for?
It records, under your signature, the authorization given to a designated person to act for your child, for a specific purpose and period.
When can it be used?
- Leaving your child with a relative during an absence
- Authorizing someone to accompany your child to an appointment or activity
- Responding to a school or organization that asks for a written authorization
Before using it
If the school, health facility, or organization provides its own authorization form, use it first. For travel abroad, use the consent for a child's travel instead. In Québec, a child aged 14 or older consents alone to care required by their state of health (Civil Code, art. 14): this authorization does not serve to consent to that care on their behalf.
