Statement of identity — one and the same person
This statement attests, under oath, that two names refer to one and the same person, yourself — for example when a document shows your name spelled differently, a name used before marriage, or your name before a name change.
It does not change your name. In Québec, a legal name change is requested from the Directeur de l'état civil; each spouse keeps their own name after marriage. The document is to be signed before a commissioner for oaths.
What is this template for?
It establishes, under oath, that the other name shown on a document refers to you.
When can it be used?
- Submitting a document that shows your name spelled differently (accents, order of names, transliteration)
- Linking two names in an administrative or immigration file
- Submitting documents issued under a name you no longer use
Before using it
This statement does not replace a name change certificate or judgment. In Québec, official name changes are handled by the Directeur de l'état civil; if the organization asks for that official document, provide it.
