Consent for a child's travel — sworn template

This statement lets a parent or a holder of parental authority consent, under oath, to a minor child's travel — by specifying the child's name and date of birth, the destination, the travel dates, the person accompanying the child, and, where applicable, the other parent's contact information.

Border authorities and carriers recommend a consent letter for a child travelling without both parents. The Government of Canada publishes an official “Consent Letter for Children Travelling Abroad” template: use it first, and check the requirements of the destination country and the airline (some require a notarized signature).

What is this template for?

It formally records, under oath, a parent's consent to a minor child's travel, with the details of the trip.

When can it be used?

• Consenting to a child's travel without one or both parents
• Providing the child with a dated, signed consent document for the border crossing
• Adding a sworn version to the official consent letter when the recipient asks for one

Before using it

Preferably use the “Consent Letter for Children Travelling Abroad” published by the Government of Canada. Always check the airline's and destination country's requirements: several require a specific form or a notarized signature.
