Statement of Stolen Document Template — Québec
This template is used to declare, under oath, the theft of a document (ID, certificate, title...), specifying the type of document, the issuing organization if known, and the approximate circumstances of the theft.
A section also lets you note the steps already taken — notably the police report, generally required in a case of theft (unlike a simple loss) before a replacement document is issued.

Before using it
File a police report before completing this template: most organizations require the police report number in a case of theft. If the organization that issued the stolen document provides its own statement form, that official form must be used first.
