Statement of gift — sworn template

This statement attests, under oath, that one person has given a sum of money or an item to another — by specifying the amount or a description of the item, the date, the relationship between the people, and the fact that the gift is final, with no consideration or obligation to repay.

It is frequently requested by a mortgage lender (down payment from a gift), by an immigration organization (proof of funds), or as part of a tax or estate process.

What is this template for?

It formally records, under oath, a declaration of a gift made to an identified person, confirming that no repayment is expected.
