Statement of Discrepancy in Information — Québec
This template is used to explain, under oath, a discrepancy between two documents that should normally match — for example a difference in the spelling of a name, a date of birth that varies from one document to another, or another administrative mismatch.
It identifies the receiving organization and the type of process involved, then lets you detail and explain the discrepancy, with supporting exhibits if needed.
What is this template for?
It attests, under oath, that a discrepancy between documents or information still refers to the same person or situation, by explaining it.
When can it be used?
- Explaining a difference in the spelling of a name between two pieces of ID
- Clarifying a discrepancy in date of birth or other details between documents
- Supporting a process where an organization asks for a sworn explanation of a noted discrepancy
