Statement of accommodation — sworn template
This statement attests, under oath, that a person is being accommodated at a given address — either that you host a relative at your home, or that you live at someone else's home — by specifying the address, the relationship between the people, and the period concerned.
It is often requested as a supplementary proof of address when there is no lease or bill in the name of the person being hosted.
What is this template for?
It formally confirms, under oath, an accommodation situation at a specific address, with the period and the relationship between the people.
When can it be used?
- Attesting that you host a family member or relative at your home
- Attesting that you live at someone else's home when no lease is in your name

Before using it
If the receiving organization provides its own accommodation or residence attestation form, that official form must be used first. Some organizations also ask for proof of address from the host (bill, lease).
