Statement of family relationship — sworn template
This statement attests, under oath, to the family relationship between you and another person — for example a spouse, child, parent, sibling, or grandparent — by specifying the nature of the relationship and the facts that make it clear (dates, names, context).
It is frequently requested in immigration, family sponsorship, estate, or file-access processes. When the organization provides its own relationship-confirmation form, that official form must be used.
