Statement of Lost Document Template — Québec
This template is used to declare, under oath, the loss of a document (ID, certificate, title...), specifying the type of document, the issuing organization if known, and the approximate circumstances of the loss.
A section also lets you note the steps already taken (police report, notice to the issuing organization, etc.), often required before a replacement document is issued.
