Declaration of care of a child — sworn template
This declaration attests, under oath, that you have day-to-day care of a child — for example for a school or daycare registration, or an administrative process where no custody judgment or other official document can prove it.
It is not a custody judgment or a guardianship deed: if a court has issued an order on the child's custody, or a mandate or guardianship deed exists, that document must be presented first.
What is this template for?
It formally records, under oath, a declaration by a person who has day-to-day care of an identified child, for an organization.
When can it be used?
- Supporting a school or daycare registration when no custody judgment is available
- Confirming your role with an organization (health, recreation, financial assistance) for a child in your care
- Accompanying other documents when the family relationship or guardianship is not obvious from the file

If a custody judgment, mandate, or guardianship deed already exists, use that official document first — this declaration does not replace it and does not settle any dispute over the child's custody.
