Statement of Residence Template — Québec
A statement of residence attests under oath to the address where someone lives, and since when — useful when no usual proof of residence (utility bill, lease) is available for the process at hand.
The template lets you specify the start (and, if applicable, end) date of residence at the declared address, with space for any detail relevant to the process.
What is this template for?
It formally confirms, under oath, a person's current or past address, when no other document can prove it.
When can it be used?
- Confirming a residential address when no utility bill or lease is available
- Attesting to a period of residence at a given address for an administrative process
- Supporting a file that requires a sworn proof of residence
Before using it
If the receiving organization (for example a government program) requires its own statement-of-residence form, that official form must be used first.
