Sworn Declaration Template — Québec
A sworn declaration is the most common format for affirming, in front of a commissioner for oaths, that certain facts are true. The declarant lists each statement as a numbered point, then swears the whole declaration is accurate before signing in front of the commissioner.
This general template fits many situations where the recipient doesn't require a specific form — but whether it's accepted always depends on the organization receiving it.

It records, as numbered and dated statements, a set of facts the declarant affirms to be true, before swearing to them in front of a commissioner.
