Statement of common-law union — sworn template
This statement attests, under oath, that two people live in a common-law relationship — usually by giving the partner's name, the date cohabitation began, the shared address, and the factors that show the couple's interdependence (finances, household responsibilities, social life).
It is often requested in support of an administrative process. When the organization provides its own form — as Immigration, Refugees and Citizenship Canada does with forms IMM 5409 and CIT 0458 — that official form must be used.
What is this template for?
It formally attests, under oath, to a common-law union and its length, for an identified organization.
When can it be used?
- Attesting to a common-law union when the recipient imposes no official form
- Describing the nature and length of the relationship in support of a file
- Accompanying other evidence of cohabitation (lease, joint accounts, mail at the same address)
Before using it
If the receiving organization provides its own common-law union form (for example IMM 5409 or CIT 0458 for IRCC), that official form must be used first.
