Simple power of attorney template
A simple power of attorney lets you designate a trusted person, the attorney (mandatary), to carry out specific tasks on your behalf: picking up a document, signing a form, representing you with an organization. You list the powers one by one and set the duration.
The document is signed by you and, to show acceptance, by the attorney.
Official form available
- Modèles de procuration — Sample powers of attorney (specific, banking, general) (Gouvernement du Québec). The Québec government publishes three sample powers of attorney. For a banking power of attorney, many financial institutions require their own form. checked on September 29, 2026
What is this template for?
It gives a designated person the power to act on your behalf for specific acts, for a set period.
When can it be used?
- Having a document or parcel picked up while you are away
- Authorizing someone to represent you with an organization for a specific process
- Delegating an administrative task during a stay abroad
Before using it
Many organizations (banks, governments) require their own power of attorney form: use it first. This template is not suitable for a protection mandate in case of incapacity, or for acts that require a notarial form, such as selling a building.
