Form 83-170SF from Indigenous Services Canada is used when a person applying for registration in the Indian Register or for a Secure Certificate of Indian Status cannot find anyone able to act as an eligible sponsor (form 83-169F).
The person must then provide two references and a statutory declaration setting out the circumstances, received before a person authorized to administer the oath.
The oath, in brief
Form 83-170SF contains a declaration under oath or a solemn declaration: it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, or justice of the peace. Once sworn, the declaration has the same weight as a statement made before a court.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
