Affidavit Template — Québec
An affidavit follows the same principle as a sworn declaration, with a more formal structure: a clearly identified purpose and space to attach numbered exhibits (Exhibit A, B, C...) referenced within the declaration itself.
This is generally the expected format when documents need to be attached and referenced from inside the declaration.
What is this template for?
It formalizes a sworn declaration that needs to reference specific documents or supporting exhibits, identified and attached.
When can it be used?
- Supporting a process where documents must be attached and cited (Exhibit A, B, C)
- Providing a sworn declaration to an organization, administrative tribunal, or institution
- Formalizing a specific declared purpose supported by documentary evidence

If the court, organization, or institution requires its own affidavit template, that template must be used first.
