A statement of facts is used to attest, under oath, that one or more specific events occurred — with the date, period, or place noted when useful. It's addressed to an organization or person identified in the document.
An optional timeline section lets you specify when and where the declared facts took place, when that detail matters for the process.
What is this template for?
It formally records specific facts the declarant has direct knowledge of, addressed to a named recipient.
