Statement of Ownership Template — Québec
This template is used to declare, under oath, ownership (or possession) of a specific item — a vehicle, equipment, or valuable item — by describing it (type, make, model, serial number if relevant) and, if useful, how it was acquired.
It can be used when no other single document is enough to establish ownership of the item.
What is this template for?
It formally attests, under oath, to ownership or possession of an identified item, along with its characteristics and method of acquisition when relevant.
When can it be used?
- Attesting to ownership of an item when no title or receipt is available
- Supporting a customs, insurance, or resale process that requires sworn proof of ownership
- Confirming the approximate date and method of acquisition of an item
Before using it
If the receiving organization (insurance, customs, other) provides its own statement-of-ownership form, that official form must be used first.
