When a person changes educational childcare service, the new provider may, under certain conditions, waive a new verification of absence of impediment instead of requiring the person to obtain a new one.

Form FO-0353 is the declaration under oath that lets the new provider grant this waiver, confirming in particular that there has been no significant interruption since the last verification.
