Form IMM 0210 is used to formally attest to the family relationship between a Canadian citizen or a permanent resident and an extended family member, and to confirm the person’s residence in Canada.
It applies to certain special immigration programs that require proof of an extended family link and of ties to Canada. Because it is a solemn declaration, it must be made before an authorized person, otherwise IRCC will not consider it valid.
The oath, in brief
Form IMM 0210 contains a declaration under oath or a solemn declaration: it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, or justice of the peace. Once sworn, the declaration has the same weight as a statement made before a court.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
