Such a declaration is often required when a discrepancy between documents could otherwise delay the processing of an application — an estate, immigration, opening an account, or recognition of a diploma.
In what situations is it requested?
- a name transliterated differently between two writing systems;
- a surname changed at marriage, not reflected on all documents;
- use of a first name different from the legal first name;
- a minor error or spelling variant between two official documents.
Does an identity declaration correct an official document?
No. It does not modify or correct any existing document — it only explains, under oath, the connection between documents that refer to the same person in different forms. A formal correction (a legal name change, for example) follows a separate process with the Directeur de l'état civil.
What should the declaration contain?
- the declarant's full identity, in all its known variations;
- the list of documents concerned and the variation observed on each;
- an explanation of the origin of the variation, if known;
- a declaration confirming it is indeed the same person.
Can it be sworn online?
Yes, when the document and situation are eligible for remote oath commissioning, by videoconference — the commissioner verifies your identification at the time of the session.
Online oath commissioning in Québec: how does it work?
Assermentation Express
Assermentation Express is not a government website and does not replace a legal name change process with the Directeur de l'état civil. This service is limited to receiving the oath or solemn affirmation on a document already prepared.
