Declaration of a lost diploma or transcript: how does it work?

Before issuing a duplicate diploma or transcript, an educational institution may ask for a sworn declaration stating that the original is lost, stolen or destroyed, and that it will not be used if you find it.

Contact the institution that issued the document first: it sets its own procedure and its fees for a duplicate or file reconstruction.

In what situations is it requested?

  • a duplicate request to the educational institution;
  • a process with a professional order or employer requiring a diploma;
  • an immigration or credential equivalency process;
  • a diploma issued abroad, a closed institution or an inaccessible file.

What should the declaration contain?

  • your full identity (name at graduation, if different);
  • the institution, program, year and type of diploma or transcript;
  • the circumstances of the loss, theft or destruction;
  • a commitment not to use the original if it is found;
  • a statement of truthfulness.
