Form 1040, “Family Status Declaration,” must be used by a student to declare their family status when they are in one of the particular situations provided for by Student Financial Assistance (AFE), or to report a change to a previously declared status.
This form does not apply to children who reside with both biological or adoptive parents. Only children from the same union may be declared on the same form — more than one form is required if the children come from more than one union. For AFE purposes, a child is considered dependent when the person has physical custody at least 25% of the time.
The oath, in brief
Form 1040 contains a declaration under oath (or a solemn affirmation): it must be signed before a person legally authorized to receive it — a Commissioner for Oaths, notary, lawyer, clerk, or justice of the peace.
In Québec, a Commissioner for Oaths can administer this oath remotely, by videoconference, when the required conditions are met (seeing and hearing each other, viewing the document, keeping it confidential).
