Sworn caregiver declaration: when is it needed and how does it work?

A caregiver provides regular care or support to a sick, elderly or dependent person. For leave, an assistance program or a benefit, the organization may ask for a sworn declaration describing the situation and the help provided.

Organizations often also ask for a health professional's attestation: the sworn declaration describes your role, but does not establish a diagnosis or the cared-for person's health condition.

In what situations is it requested?

  • a leave request to an employer;
  • an aid or benefit process reserved for caregivers;
  • supporting a disability or support application concerning the cared-for person;
  • showing the frequency and nature of the help provided.

What should the declaration contain?

  • the identity of the caregiver and of the cared-for person;
  • your relationship to them;
  • the nature, frequency and duration of the help provided;
  • a statement of truthfulness.
